sure and enter your correct birth date. c.

sure and enter your social security number. d.

sure and mark your citizenship status. 2. Complete the Uniform
Sizing form & Chart: a.

Use
the size chart to determine how to order the Class B Uniform. Because
the slacks run small, consider ordering ½ to 1 size larger than your present
size. The slacks come un-hemmed and extra long. Leave plenty of length
when they are hemmed. c.
The
instructions tell you to order the Class B Uniform once you receive your CAP
ID; however, the Squadron Commander will do that. Please do not
order the class B yourself. 3. Complete the
Emergency & Medical Information Form: a.

When all forms are completed: a. Attach check for $31.00 to CAP National Headquarters b. Attach check for $40.00 to CAP ( This entitles you to a squadron – shirt, patches and insignias) c. Submit complete package to the Squadron Admin Officer |
Senior Membership Application Procedures: 1.

sure and answer all background questions. 3. Request a CAP
Fingerprint Card from the Squadron Commander or Admin Officer. 4. Complete the Emergency & Medical
Information Form: a. CAPF 60 5.
When all forms are completed (Full Membership only): a. Attach a copy of driver’s license (front & back). b. Attach a copy of airmen certificates if applicable. c. Attach a copy of FAA medical certificates if
applicable. d. Attach a check for $76.00 to CAP National Headquarters
for full membership. e. Attach a check for $40.00 to CAP (Includes a CAP polo
shirt). f. Submit package to Squadron Admin Officer. 6.
When all forms are completed (Cadet Sponsor Members
Only): a. Attach a copy of your driver’s license (front &
back). b. Attach a check for $30.00 to CAP National Headquarters
for a cadet sponsor membership. c. Submit package to Squadron Admin Officer. |
